A car setup is the bundle of tunable parameters a sim racer adjusts to suit a track and their driving style: suspension stiffness, ride height, tyre pressures, gear ratios, aerodynamic downforce, brake bias and the differential, among others.

A good setup balances competing demands — grip versus tyre wear, stability versus agility, straight-line speed versus cornering — and is tuned to each circuit. Sim racers use telemetry to confirm a change actually made the car faster. (Setup is distinct from Balance of Performance, which is set by organisers to equalise different cars, not by the driver.)
